The 6-8 Week Baby Check (NIPE)

Written and reviewed by practising UK GPs, overseen by our Clinical Advisory Officer.

🧭 When to screen – the 6 to 8 week baby check

The 6 to 8 week baby check is a scheduled national screening consultation, not a response to a symptom. It is the second stage of the Newborn and Infant Physical Examination (NIPE) screening programme – the first being performed within 72 hours of birth – and in primary care it is usually carried out by the GP at around 8 weeks, alongside the first set of immunisations.

The purpose is twofold. First, to re-screen the four NIPE conditions – of the eyes, heart, hips and (in boys) testes – because some abnormalities are not detectable at birth and only declare themselves over the following weeks. Second, to take a broad view of the baby's health, growth and development, address parental concerns, and deliver high-value health promotion (safe sleep, vitamin D, feeding and immunisations).

A normal newborn examination does not exclude these conditions; the repeat exists precisely because they can appear later. The key clinical skill is to complete all four screening elements every time, document them, and refer promptly when a screen is positive.

System Screening for Key examination
Eyes Congenital cataract, retinoblastoma Red reflex in both eyes (dim room)
Heart Congenital heart disease, coarctation Auscultation + femoral pulses
Hips Developmental dysplasia (DDH) Barlow & Ortolani, abduction
Testes (boys) Undescended testis (cryptorchidism) Palpate scrotum – both descended?
