π§ When to suspect
Pancreatic cancer carries the lowest survival of any common UK cancer (five-year survival under 7%) and is the 5th most common cause of cancer death, largely because it presents late: nearly half of all cases are diagnosed only as an emergency. The symptoms are vague and easily attributed to benign disease, and patients commonly consult several times before referral.
The primary-care task is to hold a high index of suspicion and act decisively on the two clear NICE NG12 triggers β painless jaundice, and unexplained weight loss with associated symptoms in an older adult. Risk rises sharply with age, and smoking is the strongest modifiable risk factor.

Raise suspicion particularly in those with a smoking history, increasing age (most cases occur between 60 and 80), chronic or hereditary pancreatitis, excess alcohol, obesity, type 2 diabetes, a family history of pancreatic cancer, or an inherited predisposition such as BRCA1/2, Lynch syndrome (hereditary non-polyposis colorectal cancer) or PeutzβJeghers syndrome. Around a third of cases are considered preventable.

π§ Clinical pearl New-onset diabetes can be the earliest sign of pancreatic cancer. It may appear up to two years before a tumour is visible on imaging, so new diabetes accompanied by unexplained weight loss in an older, non-obese adult should lower your threshold to investigate β rather than simply starting an oral hypoglycaemic and moving on. |
